MAKING AN APPOINTMENT

APPOINTMENT BOOKING TIMES:

MONDAY TO FRIDAY
8:30 am to 11:30 am  and  12:45 to 3:30 pm

To make an appointment you need to do 2 things: 1) REGISTER and 2) EMAIL the office for an actual appointment date.
 

  1. REGISTER: You can register by clicking here. Please carefully and accurately fill out the registration form.  After you register, the system will provide you with a registration number which you BRING to the medical appointment.
  2. EMAIL: To make an appointment, please EMAIL us at  register@visadoctor.ca with:
  • The Names and Registration number of each individual requiring medicals
  • Telephone contact numbers
  • Country (Canadian, US, Australian, New Zealand) of immigration required
  • Your future/or previous interview date with the US consulate in Montreal - you need a scheduled interview date before you can have a medical exam.
  • Optional dates and times available for appointments. We need a week to prepare your report so please book as soon as possible.

We will contact you shortly and can accommodate quick appointments. No fasting is required for appointment.

For applicants who received a letter from the US Border Services or have a DUI charge and are dealing with a US Consulate, please contact our office via email and we will advise you as to the next steps. The fee for this assessment is $1000.

WHAT TO BRING FOR YOUR APPOINTMENT

  • Your Passport
  • Email us 1 digital copy of your photo (needs to be 500kb or less and is a headshot with slight upper shoulders against a plain background).
  • Bring your DS-260 Form
  • Letter with your Name, Case Number & Interview Date
  • A LIST of your medications (NAME and DOSAGE)
  • Please print the vaccine checklist form (see below) for your age group only and have this form completed by your family doctor or walk in clinic. We DO NOT give vaccines in our office. If the vaccine checklist is not ready at the time of the medical, you may email us your Vaccination Proof form as soon as possible AFTER your medical appointment. Proof of the FIRST DOSE in a series is only required.
  • The COVID-19 vaccines approved for use are those vaccines authorized for emergency use (EUA) by the US Food and Drug Administration (FDA) or listed for emergency use (EUL) by the World Health Organization (WHO) (see “Status of COVID-19 Vaccines within WHO EUL/PQ evaluation process”).Vaccinations received outside Canada should be shown to the doctor completing the vaccine checklist.
  • As per NEW instructions from CDC, applicants who have not received a COVID-19 vaccine dose in the 12 months prior to the date of the exam are considered due for a dose, effective as of December 08, 2023. This requirement is in place even if the primary series of COVID-19 vaccine are complete/up to date.
  • Click here for CDC's recommended COVID immunization schedule according to your age group and vaccination status (this may not apply if you have already received another approved COVID-19 vaccination formulation that is authorized or listed for emergency use).  
  • Payment in Cash and Debit or Credit Card Only. (There is a $1 charge for debit cards, and a 3% service charge fee for credit card usage.)

WHAT TO EXPECT AFTER THE APPOINTMENT

  • We will email your completed medical report DIRECTLY to the US Consulate so it will be ready for your interview.
  • In certain cases when there is not enough time between your medical and the interview date we will give a deferral letter which you take to your interview. The US consulate accepts this protocol, and it does not affect your application process.
